The engine is intentionally conservative. It identifies pathways worth learning about; it does not make treatment decisions.
NICE tirzepatide threshold
The engine uses NICE TA1026's core obesity recommendation: initial BMI at least 35 kg/m² plus at least one weight-related comorbidity. For South Asian, Chinese, other Asian, Middle Eastern, Black African and African-Caribbean backgrounds, the BMI threshold is normally reduced by 2.5 kg/m².
Primary-care NHS tirzepatide rollout
NHS England is implementing tirzepatide in primary care in phases. The initial high-priority cohort uses BMI 40 kg/m² or more, adjusted for ethnicity, plus 4 of 5 specified conditions: type 2 diabetes, hypertension, cardiovascular disease, obstructive sleep apnoea and dyslipidaemia. The tool treats this as current rollout context, not a permanent national eligibility rule.
Specialist weight-management services
Specialist services and local commissioning can use different routes from the initial primary-care cohort. NICE clinical eligibility therefore does not automatically equal immediate GP prescribing.
Bariatric-surgery assessment
The engine uses NICE NG246 referral criteria: BMI 40+ or BMI 35–39.9 with a significant health condition that could improve with weight loss, plus willingness to engage with necessary long-term follow-up. Relevant BMI thresholds are generally reduced by 2.5 kg/m² for the ethnic groups listed above.
NHS Digital Weight Management Programme
The national programme generally requires age 18+, BMI 30+ (27.5 for some higher-risk ethnic groups), and diabetes, hypertension or both.
Why we say “profile appears consistent”
A webpage cannot confirm contraindications, pregnancy status, medication interactions, psychiatric/eating-disorder context, prior treatment history, local commissioning or specialist judgement. For that reason, the engine never displays “You are eligible” as a final clinical conclusion.
Weight-loss scenarios
Scenario outputs use arithmetic only. The site does not infer that losing 15% guarantees a particular blood-pressure change, diabetes remission or elimination of sleep apnoea.
