NHS Mounjaro access in England: why the route still matters

The rollout has more than one doorway

NHS England's April 2026 guidance distinguishes specialist weight-management services from phased primary-care access to tirzepatide, also known as Mounjaro. The obesity pathway is also separate from prescribing to manage type 2 diabetes.

Primary-care access began with a prioritised group in June 2025. The 2026/27 phase expands the BMI range for people with at least four specified weight-related conditions. Ethnicity-adjusted thresholds also apply. These are clinical criteria, not a do-it-yourself eligibility test.

Why an old headline can mislead

A report saying the medicine is available on the NHS does not mean every adult with obesity can immediately obtain it from any GP. Local commissioning, clinical assessment and the treatment setting remain relevant. The current guidance also requires support around treatment, including nutrition and behaviour change.

A better conversation with your local service

Ask which weight-management pathway operates in your area, whether referral to a specialist service is appropriate and what help is available while you wait. If your main treatment need is diabetes, make that clear so the relevant guidance is considered.
