Persistent depression: what the UK pramipexole trial found

An add-on, not a replacement
Oxford reported a UK trial involving 150 people with treatment-resistant depression. Participants received pramipexole or placebo alongside their existing antidepressant treatment, with follow-up extending to 48 weeks.
The June 2025 university report describes greater symptom improvement with pramipexole, but also significant side effects. Around one in five people receiving it dropped out because of tolerability problems. Reporting the benefit without this drawback would give an incomplete picture.
What the comparison can answer
A placebo-controlled trial provides stronger evidence about the tested add-on than an individual success story. It does not establish that the medicine is preferable to every other add-on option, works for everyone or should become a routine first treatment.
The researchers called for further work on tolerability, cost-effectiveness and comparisons with alternatives. The announcement is research evidence, not a new general prescribing instruction for readers.
When existing treatment has not helped enough
Our suggested conversation with the treating team is about the whole history: which treatments were tried, how long they were used, what changed and which adverse effects were difficult. Ask whether a specialist review is appropriate and how the next option would be monitored.
Bring questions about the study if useful, but do not add, stop or alter medicine on the basis of an article. A trial involving selected participants cannot determine personal suitability.
The value of this UK result is that it expands the evidence available for a difficult clinical problem. Its limitations and treatment burden belong alongside the positive result, not in small print.
